Prevention Case Management

Consejo’s Poder es Salud HIV Prevention Case Management Services Program is client-centered with the fundamental goal of promoting the adoption and maintenance of HIV risk-reduction behaviors by clients with multiple, complex problems and risk-reduction needs. Poder es Salud is designed to meet the needs of Latinos who are HIV+ and their partners who are having or are likely to have difficulty initiating or sustaining practices that reduce or prevent HIV acquisition, transmission or reinfection.

Poder es Salud provides highly intensive, ongoing, individualized HIV prevention counseling, emotional support and linkage to community resources. All activities address the relationship between HIV risks and a variety of other problematic issues affecting clients such as substance abuse, other STD’s, mental health issues, language barriers and the many social and cultural factors that affect the Latino community.
