Case Management

Consejo’s HIV/AIDS Case Management Program provide clients with a range of services including support in meeting basic human needs such as nutrition and housing, individual and group therapy and support, HIV Prevention Case Management, mental health services, medication management and advocacy while accessing and applying for medical care and HIV programming available through community health agencies as well as linkage to community resources as needed.
